I certify that all statements (verbal and written) made on any and all material collected during the application process are true, complete and accurate and I understand that misrepresentation or omission of facts called for in the volunteer application, resume, interview process or other application material may prohibit consideration for volunteering at Shotgun Manor and is cause for immediate dismissal.

Reference and Background Checking

Applying to be a Volunteer at Shotgun Manor allows us to contact any of your schools, or other references for the purpose of verifying information and/or obtaining an account of your education, work experience and skills. By applying for a volunteer position you agree to hold any and all of your reference sources harmless and free of any liability for releasing such information.

Testament of Commitment and Presents

I certify that if chosen, I will be at Shotgun Manor on the Dates that I have requested. If additional dates arise that I would like to request, I will inform Shotgun Manor at least 3 days in advance. If not, I understand that I might not be allowed to participate in that evenings show. Parental Testament

(If under 18) I certify that I am giving my son/daughter permission to volunteer at Shotgun Manor Haunted Attraction. I understand that this is a community event and that no payment will be made to my son/daughter. I also certify that I have reviewed the application and all statements made are true, complete and accurate. I also give my full support to my son/daughter and will do MY personal best to encourage them to arrive in a timely manner as well as understand that Shotgun Manor does not end until the last patron is out the door and the staff meeting is complete. I also understand that this commitment by my son/daughter is one of trust and that I will do everything possible to encourage them to complete every night that they have committed to working.
